I keep getting the following message when trying to login to my newly created account:
Login failed - AUTOSUSPENDEd
How can I regain access??

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Heading" has fields like "Login & Password" which makes it suspected of being used for phishing.
Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the suspected fields from yoru form and activated your account again.
Kindly refrain from creating form with email/password, login/password or credit card details.
