Configure custom shipping charges on form

Hello! I would like to add shipping charges to my form "Centennial DVD".  How do I do so based on the number order?  1-3 ordered is one price based on priority shipping costs. 4+ is a different price.

Unfortunately, we do not currently have an option to set different prices based on the priority and quantity ranges.
