Hi there, my form/account seems to be disabled. Please help!

    I see that the account is suspended due to the form http://www.jotform.us/form/70554328373155 which is collecting SSN number. Please be noted that it is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    It is required to delete the guilty fields on the form in order to activate your account. Please confirm so that we can delete the field from the form and activate the account.
