How can I create a form to consolidate availability across 3 months

I'm looking to create a form to consolidate the availability of respondees over a span of about 3 months. The form should fulfil ALL of the following:

1. Allows respondees to select unlimited dates from a calendar
2. After a respondee has selected a date, the next respondee can still select the same date (i.e. we will consolidate 2 availability for that particular date)

Optionally, if the form can also fulfil the following, that would be great:

1. Respondees are not able to select Sundays
2. Respondees can only choose the dates in a specified range of months (e.g. only June to August).

Thank you for waiting. While your other requirements will work in the Date Reservation widget and in the appointment field, it is not possible to select multiple dates on every submission. As a workaround, you can use the Configurable List widget that could be set up with multiple date fields. Please see the screenshot for your reference:
