Account Autosuspended: due to form phishing

Thanks for reaching out to Jotform Support. I’m really sorry that this is happening. We have checked your account, and found out that it has been auto-suspended by our system due to your Appointment Request Form having the word "Password" in a certain field. 

Do note that if you use the word password or in your forms, your account will be flagged and have a higher chance of being suspended again by our phishing filter, because it is against our Terms of use. Do refrain from using that word in your forms. We have deleted the offending field, reactivated your account and add your IP to the whitelist.
