Create survey template from Excel

Hi there,

I have created a mental health survey and all its questions.
Do I have to recreate it in Jotform or is there a way I can create the form by importing from Excel

Thanks for reaching out to Jotform Support. Unfortunately, you cannot convert an excel file into an online form. You can, however, convert your excel sheet or print it as PDF, then import the PDF document using Jotform's Smart PDF Form. Let me walk you through on how to Convert PDF as form.

• Click the Create Form button on your My Forms page. 
• Choose Import Form and then Import PDF Form.
• Now, upload your PDF file.

• Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.

Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that: 

• Finally, click on the Wand icon below the Adobe Sign Widget to open Widget Settings.
• Now, in the Adobe Sign section, on the right side of the page, click on Authenticate to authorize the widget.
