Why is our account suspended?

    Hello, I am writing because my JotForm became disabled, and my account is suspended when I try to log in.

    I'm contacting to find out why the account is suspended, and what I can do to reactivate.

    I have checked your JotForm and see that it was reported due to the Job application form using a different company logo and the account email address is using a public domain. Please refer to our terms of usage for more information. In order to activate your account, it is required to remove the guilty fields on the form and change the account email address with your official email address.
