I believe the owner of this page is trying to scam personal data from me and I would like all of my sensitive data removed from your website. I believe the owner has used a job scam to collect information. The business seems to be nonexistent as it is not listed on any major business search site.

Jotform is an online form builder that many webmasters use to build a different kind of forms. However, some phishers might use our tool to make forms where they offer jobs, or they request credit card or critical/personal information such as SSN or login credentials.
They will reply to you on this thread once they inspect the form and the account.
It would also be helpful if you can tell us how did you find that form. Was the form send to you via an email or did you find it on the web? If you found the form on the web, please provide us the webpage URL.

In order to further assist you we will need the website where you filled out the form.
That way we can trace back it's usage.

It was received via email after I had a few job websites emailing me and a few different people emailed me with more information. This person however was not on the job listing websites. I checked back after I filled out the application.

I checked with my colleague ktyaguirre and she told me that unfortunately there is nothing we can do if the form was shared via an email. We can only check if the form is embedded on a website. You would have to provide us the website so that we can trace back the user.
