Need help reactivating suspended account.

    Hello,

    I logged into my business account today to utilize a template and my account is suspended. Not sure why. I don't care if you shutdown my personal or the old business account but Please reactivate the Luvv Creates Memories Sweetly Made account.

    As you pop-up states that the forms asked for personal information, as they should as I sale baked goods and would need to be paid for my services. If whomever was trolling so well looked they would see that the forms were not used by anyone. I only started my business when I was creating forms using your forum. I decided at that point to only use your site for internal business needs, because I wouldn't want to jeopardize my customers information.

    This is my Legit Business with the proper State and Federal Documents showing it is legit. Thanks I would appreciate it if we can have this resolved ASAP. We all know how important time is. This day in age time is money.

    Thanks for reaching out to Jotform Support. I am sorry you are having an issue with this. I've checked your account and I can see that it is currently suspended. This is due to the the Request for Taxpayer Identification Number and Certification suspected of phishing activities. We know this might not be the case with you. Can you please provide us with below information:

    1. The URL where your form/s is/are used.
    2. Provide any document that will support your claims.
    3. Your website or proof of affiliation to any banking institutions.

    Send all of these to reviewers@jotform.com with the Thread # 14306851 as the subject. One of our colleagues will get back to you about the status of Your account.
