    Subject: Urgent Request for Account Unlock in JotForm

    Dear JotForm Support Team,

    I trust this message finds you well. I am reaching out to you regarding the recent and repeated blocking of my account on JotForm, despite not having used it during the period in which the block occurred.

    Unfortunately, I have noticed that my account has been blocked again without apparent reason or prior notification. It is important to note that during the period when my account was previously blocked, I did not engage in any form of activity or access to the JotForm platform. Therefore, it is both perplexing and concerning that my account has been blocked again without any clear justification.

    As a loyal user of JotForm, I rely on this platform to manage critical aspects of my business, including sales registration and the collection of important data. The ongoing disruption caused by the blocking of my account has significantly impacted our ability to conduct these activities efficiently and in a timely manner.

    I understand and respect JotForm's security measures and usage policies aimed at protecting the integrity of the platform and ensuring the safety of all users. However, I kindly request that you review the status of my account and proceed with its immediate unblocking.

    I am committed to adhering to all of JotForm's policies and guidelines and am willing to cooperate in any investigation necessary to resolve this situation properly.

    Thanks for reaching out to Jotform Support. Your account has been suspended due to the collection of credit card information on this form: 241134927704051.

    Please note that the collection of sensitive information such as credit card information, login credentials, etc, is violating our Term of Use.

    You may not use the Jotform to collect, use, or disclose personal information protected from any of these activities under privacy laws. You further agree to not use the service to collect, use, or disclose credit card information or any type of login credentials. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    To reactivate your account, you will need to comply with our Term of Use and allow us to remove the offending fields. Would you comply with our Term of Use and allow us to remove the offending fields?

    I hereby confirm that I will immediately remove any fields containing sensitive information, including credit card details, from the form in question (241134927704051). Furthermore, I commit to utilizing Jotform solely for legitimate purposes that are in accordance with your Terms of Use and applicable privacy laws.

    Please proceed with the reactivation of my account, and rest assured that I will take all necessary measures to ensure ongoing compliance with Jotform's policies.
