I am receiving this message when I attempt to submit the form:

There are errors on the form. Please fix them before continuing.

Thanks for getting back to us. I checked your draft, and the information you entered in each field looks to be correct. However, please that in order to finish the form, you must also enter your credit card information.

It didn't work initially, but when I went back to the previous page it looks like there was an acknowledgement box that wasn't checked and that seemed to do the trick! Thank you for your help!
