Can I create a an auto populated "work order" form based off the submission of an online order?

Basically, I want customers to fill out a form. After they submit it, they'll see a thank you page. At the same time, a second form will be filled out automatically with some information from the first form. This second form will be available on the dashboard for the delivery driver to use. The driver will use it to get a signature and a photo from the customer.

If this is not possible, is there are work around?
