    Why is my form unavailble unless i upgrade to a HIPAA compliant account when i dont need it ? Need my form reinstated ASAP.

    Hi Katie,

    Thanks for reaching out to Jotform Support. I'm sorry that this is happening. Our system detects that you're dealing with Personal Health Information. As required by laws within the US, everyone dealing with healthcare data should be under HIPAA.

    But, If you are not a HIPAA-covered entity, please follow the steps below:

    1. On your My Forms page, hover your mouse on your Profile Avatar icon.
    2. In the window that opens, click on Settings
    3. Click on the Data tab on the left side of the page.

    4. Then select NO, I'm not a HIPAA-covered entity in the modal.

    After selecting that option, you can now enable your form. Let me show you how:

    1. Open your form in Form Builder.
    2. In the orange navigation bar at the top of the page, click on Settings.
    3. In the Form Settings window, set the Form Status to Enabled and click on Save.
