I would like to know why my account is suspended and how we can resolve this

    I would like to know why my account is suspected and what I can do to resolve this situation.

    The account was suspended due to requesting social security and drivers license numbers through your form.  More information regarding collecting SSN's can be found here:

    If you would like, we can remove those fields for now and activate your account.

    Even with terms of use and a signature, we still limit who can request this type of information. 

    • Insurance companies

    • Credit card companies, lenders, and any other company receiving a credit application from you

    • The three main credit reporting agencies: TransUnion, Equifax, and Experian

    • Any company that sells products or services that require notification to the IRS, including:

    - Investment advisors

    - Banks

    -Real estate purchases

    -Financial transactions over $10,000, such as automobile purchases; and other financial transactions

     If your purpose is not to collect SSN from the general public, but from your own customers; and if your business is among the list above and you comply with recommendations below, then we can make the exception.  If not, we can remove the fields.

    I already removed the guilty fields so you can continue using your form, you may request the basic information through the form and ask them to send you the rest through another method.
