The connection to submit.jotform.us was interrupted while the page was loading.

    The page you are trying to view cannot be shown because the authenticity of the received data could not be verified.

    Please contact the website owners to inform them of this problem.

What is his problem?

The error seems to be related to security. The users browser may have blocked submit.jotform.com, kindly  tell your user to disable any pop-up blocker or whitelist submit.jotform.com in his internet security.
