Does jotform have a dynamic feedback prompt?

I am trying to build a feedback form where the 5 star rating dynamically provides prompts associated with the rating. For example, I want 1 star rating to pull up negative “pills” as prompts (dirty / poor service / unclear communication / poor lesson content) and 5 star ratings to pull up positive “pills” like (clean / good service / clear communication / good lesson content). I tried the conditional formatting, but could not get from my star rating to the correct multi select form section.

Please advise.

Thanks for your patience. To add a feedback to rating system, you can set up a Star Rating form element with a conditional logic to show the feedback. Let me show you how.

1. On Form Builder, Click on Add Form Element.
2. Add Star Rating to your form.
3. For your feedback you can add Paragraph elements to your form.
   
   

Set up your Paragraph field with your feedback, then hide the fields. Here's how to do it:

1. Select the Paragraph field.
2. Click on the Pen icon.
3. Put in your feedback, then click on the Check icon.
   
   
4. To hide the field, select your Paragraph field, click on the Gear icon, click on Advanced, then Toggle Hide Field to On.
   
   

Add a conditional logic to your Star Rating field. Let’s do it:

1. On Form Builder, click on Settings on top.
2. Click on Conditions on the left.
3. Click Show/Hide Field.
4. Put in the following logic.

• IF - the Star Rating field
• STATE - Is Equal To
• TARGET - Value
• VALUE - 1
• DO - Show
• FIELD - The poor service Paragraph field.
  
  

Do the same for the other ratings and feedbacks.
