Account Suspended: Fake service

    Hi, I am waiting on a response to my support ticket about my account being suspended. After reviewing prior posts I realize a form I created is in violation of the terms of use. Please delete the form or remove the fields that are in violation so that I can gain access to the account. 

    When can I expect a reply? This is my second email.

    Thanks for reaching out to Jotform Support. I've checked the other thread and can see that my colleague advised you to contact the relevant team to reinstate your account and provide the information below:

    1. The URL where the form is used.
    2. Provide any documentation that will support your claims/legitimacy.
    3. The website or proof of affiliation to legitimate institutions.

    Please this information to reviewers@jotform.com with Thread #14229201 as the email subject. One of our colleagues from the Reviewer team will get back to you about the status of your suspended account.
