Form is being used for wrongful purpose.

Someone that us ckaiming to have a pet transportation company is killing animal and advertise it as a buisness on your side. You need to take this buisness off your page so they stop killing pets!!! This buisness is calked nates pet transportation

They are acting like they are a rescue/pet transport company but dont follow any law or regulation on their transports. To get more money they put too many pet carrier in their van so the animals are suffocating with no air flow in the transporter. 6 dogs died in the last few weeks in their care. Under the #Justiceforopal you can find proof and more info on social media. We just need to stop them so people dont use their transport service anymore

After looking into this, we determined that the form creator violated our terms of use. So, we immediately suspended the account, the account owner, as well as any forms and apps connected to it. But, keep in mind that although the form owner has no access to the form or data at Jotform now, they might have received your submission information if they enabled the Notification Email feature.
