Unable to submit form, it gets stuck on "please wait".

Hi I am trying to sign up on the page

But when I go to confirm everything and click "review entries before submitting" the page just sits there saying "please wait" and I can never confirm the time I want. I am trying to sign up for the time 3:30- 3:45 my name is Theresa Daniel and my email is theresadaniel.cb@gmail.com and my phone number is 412 758 5797
