I created a calendar widget. When people were checking dates on the calendar, they noticed that some of the dates weren't allowing them to click on it. So I went back and deleted that calendar and created a new one, in which the dates worked properly. But now when the form is accessed, both calendars show up - the first one that doesn't work properly, and the second one that does work properly. I've now deleted all calendars from the form, but they both still show up.

I checked your form and I can only see 1 Date Picker tool. Here's a screenshot:
