HTML and jotform combination not working

    Ive created a swatch request with checkboxes and images, and have wrapped it within a jotform that has the information fileds, captcha, and submit/reset buttons.  I've downloaded the sorce code, moved the CSS and JS files to my directories, corrected the paths on my html page and pasted the jotfom code, wrapping the <form> element around  my html.

    Here is the link to the page:

    I have 3 problems.

    1. The hint example text is not showing up in the 2 text area boxes.

    2. The captcha button is not working properly.

    3. I have wrapped the jotform around my checkbox inputs, but when I submit the form, it only generates the jotform fields, not my checkbox values.

    Yes, I figured that there was a script conflict, but I also thought as you are more familliar with your scripts you amy be able to point out the conflict.

    The only reason I made the form this way is, I couldn't see how I could get a jotform to look like the part of form that I coded manually using jotform.  I am new to your site so any ideas on how to recreate the this form fully in jotform would be appreciated.

    Is this something that I can do with that iFrame that you mentioned?  How would you go about making a swatch request form like this in jotform?

    Hi Ana,

    1 & 2 - You have incorrect path of your JS files, this maybe the reason why JotForm doesn't function well. As of this typing you are using js/js-swa instead of js/jot-swa check this screen. Kindly review.


    3 - Like what my colleague have mentioned, you do not have this on your form so it's not possible to get it's responses via Email except for the fields created on JotForm. The best way to overcome this is to create these checkbox fields to JotForm and after that, get the form source code and customize it in any way you want. Just be sure not to alter any of the checkbox field ID's to avoid missing answer on submissions notification.
