I'm attempting to log in to my account. I normally leave myself logged in, so I'm not sure why I was logged out in the first place, but I am. I clicked on "Forgot Password?" and entered my e-mail address (email@example.com) and was told that there was no user with that address in the system. I'm positive that's the correct address, as I know that that is where my form responses go to.

As I've check the email address that you provide and I found one account associated on that, which is a guest account. I would suggest you to Register to free account and use the same email address that you used on your guest account so that all your data and form would automatically transfered.
