Hello,
I have been trying to log in for 2 days and it will not allow me to do so. I have even tried changing my password and still cannot log in. Is there away someone can help me with this or tell me what I need to do to be able to login? It says login suspended when I try.

As you already know, your account was suspended. And the form that triggered the suspension is this form — Travel Disclaimer and Insurance Waiver
The form is collecting credit card information which we do not encourage our users to do to be compliant with our terms.
Also, please know that JotForm has an anti-phishing system that scans forms and suspends accounts that own suspicious forms.
In order for me to reactivate the account, I have deleted the following fields in your form.
If you want to collect payments using your form, I suggest that you check out the available payment integration that we have.
