My account is no longer accessible to me. What do I need to do? I have forms in this account that are not located anywhere else.

I have checked your account info233 and it has been suspended due to being reported as suspicious of phishing activities,your form "application -volunteer current" is the one that triggered the suspension.
It has fields collecting sensitive information and that violates our -Terms-of-Use
In order for your account to be activated again, we need to remove the guilty fields.
We will wait for your response.

This is a ministry account and the people filling out these forms are notified that this information is collected from them in order to do a background check in preparation for applying for their permits when they enter the foreign country -- Botswana.
Prior to this, our forms were housed on a jotform secure site so I am not understanding what is needed from me next. Is there some security aspect that I must include that is not provided by jotform; if so, how do I do that? Thank you.

We encourage you to follow this recommendation:
Add a sort agreement and/or an e-signature field to the form, so your customers will agree to provide their SSN along with personal info.
