For this form, if I edit it, I can see all of the fields the user is supposed to fill out. However, when I look at the "published" version, those fields are not visible.

This shows the published version how these fields are empty:

    Hello Nathan,

Thanks for reaching out to Jotform Support. I'm sorry you're having trouble with this. When I cloned and tested your form, the same thing happened for me. After investigating it a bit, I found a solution. We just need to disable the condition#7 as it will hide those fields if the certain value was not selected. Let me show you how:

    1. In Form Builder, on the orange navigation bar, click Settings.
    2. Go to Conditions tab and hover your mouse over to condition #7 then click the gear icon.
    3. Select Disable.

Take a look at the results below:

Give it a try and let us know how it goes.

Thank you! That seemed to fix it!
