Trouble filling out form follow up

    I couldn't find previous conversation.

    The website is

    Then look down to. Annual Recertification Packet

    I used my phone and it worked fine.

    It still does not work on my tablet. I have followed all your suggestions to no avail. If no one else has brought it up it must be something in my tablet.

    Thanks for all your help but since my phone was able to send it I am done and you probably shouldn't try to worry bout it.

    I have also check, and it looks like the email address you used within this thread is not associated with any Jotform account too. If you have an issue with your form, could you share with us the form URL so we can help you more to it?
