MY BKG FORM IS DISABLED . PLS ADVISE WHY

I have checked your account and I can see that you have one form in My Forms which is working correctly.
You also have two forms in the Archive folder.
To enable those forms, first, go to the Archive folder (1). Then select the forms that you want to activate and click Unarchive (2). The forms should me moved to the My Forms folder now.
Make sure that the forms are Enabled by clicking the More option (3, 4).
