Create a form for coaches to evaluate their players

I would like to use Jot form to create one form that I can send to multiple coaches in our basketball league so they can evaluate players performance. The results of the evaluation will be used to help coaches pick players and keep teams even next year.

I would like the coach to pick their name from a drop down, then the coach can select a player name from a pul-down menu for each row.  They will then enter 1-5 in each cell on the player's row.

Is there a template I can use to start from?

Or, start creating your form. We can help you create the form, but I have a question whether each coach has its list of players and if coaches shouldn't be able to see players from their co-coaches.
