Form User: Did not receive the certificate.

Hello, I hope you're doing well! I attended the Pet First Aid training session at the Seattle Humane Society in Bellevue last Saturday, April 27th. The instructor mentioned that we would receive our certificates via email by Tuesday, but unfortunately, I haven't received mine yet. Would you be able to assist me with this?

Thanks for reaching out to Jotform Support. I am sorry you are having trouble with this. Unfortunately, we can't send you your certificate as that data is being managed by someone who created the form using Jotform. You will need to reach out to your instructor and ask for your certificate.
