Smart PDF Form: Data is not showing in PDF

I am working with https://www.jotform.com/build/241217163633147

I need help with two issues.

1. Only some of the fields from the form carry over to the PDF. It appears the fields are all linked to the PDF, but it's not working.

Thanks for reaching out to Jotform Support. The Unique Name of the Name field is not the same in the Paragraph element which is why it's not showing in the PDF. You need to update the Unique Name. Here's how to do that:

1. In Form Builder, select the Name field and click the Gear icon.
2. Under the Advanced tab, click on Field Details at the bottom and copy the Unique Name.
3. Paste the Unique Name in the Paragraph element.
