The form intermittently submits successfully, but many users are reporting the same error as above. Is it possibly related to browser dependent issues / or cache related?

Apologies for all the troubles caused. I have tested your form and I was able to replicate the issue. There were some submissions that we successful and others were not.
I have escalated your issue to our back-end team for further analysis and resolution. We will update you on this thread once issue is resolved.

@rstein you may want to check out with Asana as well if you're having a forbidden error. The user from this thread found out that it has something to do with their project.
