Why my account was suspended for Phishing?

HI, my account was just suspended. The reason is not accurate. I am trying to create an app for my cleaning team to be able to log in and see cleaning available jobs and have them fill out forms as they complete each job.

Thanks for reaching out to Jotform Support. Your account has been suspended because it is suspected that your Form was used to collect login credentials. Note that collecting such information in Jotform violates our Terms of Use. I understand you're not collecting this information, but our auto-phishing detector will detect the terms "username" and "password" in the field label. So I deleted those words from the field. You can use different words to let your form fillers choose their personal code.
