Form Locked: required to upgrade to HIPAA

    My jotforms say I need HIPAa upgrade required!

    Please help don't know what's going on

    Hi Wendy,

    Thanks for reaching out to Jotform Support. I'm sorry you're having issues with your form. After checking, it seems that the system have identified your form as HIPAA-compliant. If this is not the case, please try to do the following:

    1. Log out from your Jotform account and clear your browser history.
    2. Then, log back in, and you should then see the modal below.
    3. Next, click the Go Back button.

    4. You should see the modal below, select No, I am not a HIPAA Covered Entity.

But, it may take a few minutes to take effect.

    I followed the instructions as you mentioned and my forms are still unavailable


    Please help

    I did that several times.

    It seems like that warning signs for the HIPAA is gone. But all my 3 forms are unavailable for my clients to see and fill out.

    Hi Wendy,

    Thanks for reaching out to Jotform Support. It looks like the form got disabled through form settings. Please try to enable it through Form Builder. Let me show you how:

    1. In Form Builder, click on Settings on the top orange navigation bar.
    2. Under Form Status, click on the dropdown box.
      Form Locked: required to upgrade to HIPAA  Image 1 Screenshot 20
    3. Select Enabled from the dropdown and click on Save.
