My Account Is Suspended

well i were afk and my account got SUSPENDED???
like what happend?

oh XD
never tought jotform would suspend me for making an login way
not even an real kind of account
only to make the forms so i knew who made them XD
next time include that to the terms or smth
cuz now i think its just dumb XD

Upon checking your form, it appears that this is due to the Username and Code fields in the question: before you continue, type the username and code given by your operator.

If this is not a login credential, we recommend using another term for collecting this information, as it will be flagged by our anti-phishing system. To reactivate your account, we need to remove this offending field.

but isnt jotform also used for companies?
if yes then this is like your name if you think about it
+ if you look again its not ur personal info
and if you look more closely to the forms you will also see that the usernames and codes are like encrypted inside jotforms
so basicly its not real card info or smth

Thanks for getting back to us. The fields are still tagged by our system as collecting log-in credentials, which is prohibited as per our Terms of Use. Since the form has no submissions yet, I've removed the offending fields and reinstated your account. You should be able to log into it now.
