Why won't my form submit?

Every time I try to submit my form it brings me back to the first page and does NOT submit the form. I have tried multiple times. 

Are you still experiencing this problem since when I tested your form, I wasn't able to replicate that issue:

After further investigation, the original form layout of your Form is Card Layout correct?

The form will not submit when it is not in Card Form Layout.

If you want to use the Classic form layout then you need to remove the submit button

and add it again.
