How to add a Middle Name?

Currently this form asks for First Name and Middle Name.

How can I add a third field for "MIDDLE NAME" to this??

Thanks for reaching out to Jotform Support. Adding a middle name in the full name element is easy. Let me show you how:

1. In the Form Builder, click the Gear icon of the Full Name element.
2. Go to the Options tab.
3. Enable the Middle Name.
