Thanks for reaching out to Jotform Support. I’m sorry that this is happening. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
Have you tried integrating your form using a different device or attempting the integration using a desktop? Meanwhile, I also cleared your form cache to see if that helps to resolve the issue.
