When downloading to pdf, questions in the wrong order, all wrong information

When I download survey responses to PDF, they will often download with incorrect headers (from different surveys) and missing and wrong information/questions. It's almost as though two different surveys are downloading and being mashed together into one PDF. When I click print and then download, the correct questions and answers transfer to the PDF, but some of the survey formatting is lost.

In the attached screenshot, you will see part of the downloaded pdf of a survey. This survey did not contain the question "Do you agree to replace your current serum with product provided by media lab" and yet that question shows up in the downloaded pdf.

This is quite frustrating and strange and has been going on for over a month. I'd love to be able to discuss the issue over the phone with someone.
