Account Suspended: Collecting Login Credentials

Please review my banned account. My contracts are for fantasy roleplays.

I checked your account, and saw that it's suspended due to this form, due to collecting of Login Credentials (Username and Password). Online Form Builders, including Jotform, are sometimes being used for identity theft, and we go to great lengths to prevent this. That is why collecting these types of data is considered a phishing activity, as stated in our Terms of Use.

To re-enable your account, you need to confirm with us that we can remove these offending fields.

Once we hear back from you, we'll be able to move forward with a solution.
