Hi team,
We renamed a form and it does not change when we send it out. We duplicated it again with the new name and still keeps the original "vaccination records" title in preview when sent over text. This complicates things for us because we have to open the form every time to ensure we are sending two different forms and clients think it is the same form sent twice so they don't fill out both.

Thanks for reaching out to Jotform Support. I’m sorry that this is happening. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. When I looked at your form, I saw that the Page Title wasn't configured correctly. Let me show you how to fix it:
- In Form Builder page, select Settings located at the top orange menu bar.
- Under Form Settings, Click on Show More Options.
- Scroll down to Page Title.
- Lastly, update the Page Title with your desired title.
