    Hi Mike,

I can see you've posted 4 different questions (questions: 1, 2, 3, 4) related to header and subheaders. Will be answering them here since they are closely related.

Upon form creation, a header is automatically added to your form with the form title as the header.

To add a new header, click on Add Form Element.

Select Header and drag it to your form.

To change the text, click on the newly added Header and change the text directly. You may also change the subheader text by clicking on the line right below the header.

Likewise, you can change the header and subheader text by clicking on the gear icon. It will open up the "Header Properties" panel on the right side. You can then update the values inside "Heading Text" and "Sub-Heading Text".

To change the alignment, scroll down on the Header Properties panel then under "Text Alignment" choose either Left / Center / Right. Note that the alignment will be the same for the Header and the Subheader texts.
