My account was suspended and the form is gone

    Hi,

    A few days ago I spent a long time creating a form. After this, I found that my account had been suspended. Just a few hours ago, one of your team helped to unsuspend it. However, now, the form I was creating has completely vanished from my account. Was the form deleted at the same time as the account was suspended?

    I have had a terrible experience so far with Jotform. I will be writing a review about my experience. Please reinstate my form on my account - the name of the form was Nirmala's pickles.

    Hi Mitali,

    Thanks for reaching out to Jotform Support. Unfortunately, it is not possible to recover the deleted form. Your form#241241791864056 was deleted by my colleague because it was violating our Terms of Service. You can create a new form and make sure that you are not violating our terms by using copyright logos. If you want to use a copyrighted logo, you will need to provide the following information:

    1. The URL where your form is used.
    2. Provide any document that will support your claims that it is your organization logo.
    3. Your website or proof of affiliation to legitimate institutions.

    Send all of these to reviewers@jotform.com with the Thread#14584181 as the subject. One of our colleagues will get back to the customer about the status of their account.

    Can you kindly provide me with the evidence about which copyrighted logo I used which was violating your terms of service?! I was making a form that was to sell my grandmothers home made pickles. On the product pictures, (WHICH FYI I TOOK MYSELF!) there were labels, which I have designed myself, on my computer, so I really don't understand what copyright issue you are referring to. This is getting more and more unreasonable and ridiculous by the minute! I would be most grateful if you could reissue the form which I spent time making. I will certainly be writing public reviews about the ridiculous nature of this, and informing people to stay away from Jotform. I request you to please return my form to me.

    I also require a written apology for making false statements about my work, and claiming that I have violated your terms of service, when this couldn't be further from the truth.

    Thanks for getting back to us. I understand how upsetting this is. I’m going to do my best to resolve it for you. Unfortunately, as my colleague mentioned above, we can't able to recover the form, once it totally deleted from the account. I have forwarded your concern to our Relevant team for more information.

    Hi Mitali,

    Thanks for your patience and understanding. We appreciate it. Your account was flagged by our system and placed on hold while it was reviewed by our reviewer team. Which why my colleague Arslan suggest to send us the URL where your form is used, and Provide any document that will support your claims, as well with the website or proof of affiliation to legitimate institutions. Then, send to our reviewer team to further investigation. Since your account is activated right now, you can now create a new form or use one of our template.
