Create form sections

I would want a problem section called "Chair", "Keyboard and Mouse", "Monitors", etc

Thanks for reaching out to Jotform Support. It is easy to create sections in your form. Let me show you how:

1. In your Form Builder, click on + Add Form Element on the left side of your page.
2. Under the Form Elements, go to the Basic tab and add a Section Collapse to your form.
3. Repeat the same steps to other sections.

That's it! Test your form, and you should see that your questions are grouped based on the assigned section. Check out my screencast below for the result:
