Jotform/Asana integration not working

I am trying to set up an integration between Jotform and Asana. It was working, but then a few days ago it stopped, and I can't figure out what happened. i have tried setting up several integrations to the existing project, and to a new project just to see if it was not aligned and neither has worked. When I submit a jotform, it never comes through in the Asana project.

Any ideas?
