Asana integration: White screen after clicking submit button

We are creating a test form to make sure everything works well with our asana integration before we purchase a higher tier but we can't get the form to submit any data when clicking submit.

I'm not sure if we overlooked something or not. We just get a blank white screen when hitting submit.

I've reviewed your Asana integration settings and saw you have not setup any "Match Your Fields" data.

This setting will tell Asana, which field they will get the value from that will be displayed on their end.

I've cloned your form and setup the integration with the following settings as an example.
