Why I have been banned

I'm lecturer an try to teach my students how to use jotform but I got banned, could you remove the banned please?

Thanks for reaching out to us for help. I'm sorry you are having difficulties accessing your account. Upon checking, I found that your account is suspended because you are collecting password information on the form 241272361532449, which is against of Jotform Terms of Use.

To activate your account, the offending fields need to be removed from your form. Once It is removed we can go ahead and activate your account. If you allow us to do that, we'll go ahead and delete the offending field and activate your account.

Once we hear back from you, we'll be able to move forward with a solution.
