Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screencast below to see my results: 

To troubleshoot, could you please try re-embedding the form using the iFrame code instead of using the default Jotform embed code? By utilizing the form within an iFrame, the risk of script conflicts on your page is significantly reduced. Let me show you how:

1. In Form Builder, click on Publish in the orange navigation bar at the top of your screen.
2. In the menu on the left side of your screen, click on Embed.
3. Click on iFrame then click on </> Copy Code.
