Account suspended

    Thanks for reaching out to Jotform Support. You're account has been suspended due to login credential collection such as passwords, secret answer, etc., which is not allowed according to our Terms of Use. In order to reinstate your account, please provide the following information:

    • The URL where the form is used.
    • Provide any documentation that will support your claims.
    • The website or proof of affiliation to legitimate institutions.

    Send all these informations to reviewers@jotform.com with Thread #14657203 as the email subject line. Our reviewers will get back to you about the status of your suspended account after they've thoroughly reviewed these informations.

    The form is used only for collect information regarding our clients help with application.

    Hi Diana,

    Thanks for getting back to us with these details. Our Terms of Use don't allow login credential collection and to collect any personal information such as the national insurance ID, etc. if you are not a legit organization to collect such data. If you think that your account has been suspended by error, kindly proceed as explained above.
