Can I add repeated survey fields to let users add a new entry?

    For example, I am asking about project location. I have three questions about project location.

    1. At what scale does the program/project operate?
    2. Location
    3. Name of watershed - If in the US, please refer to 8 or 12 digit HUC

    I'd like to add functionality so that users can add multiple other locations. Is there a way to do this?

    I found a way to use conditional logic to ask: "Would you like to add another location?" and repeat questions 1-3 if their answer is yes. But that would only happen a finite number of times (e.g. I could manually program that loop 3 times, but if the user had a 4th location they would not be able to add it).

    Is there another solution? Could I ask a question like "How many more locations would you like to add?" and then have questions 1-3 pop up that many number of times?

    Hi Mia,

    Thanks for reaching out to Jotform Support. The hide/show conditional logic that you have in mind also works for your goal. However, another alternative is to use the Configurable List widget. This widget allows you to add multiple fields and the users have an option to add a new set of those fields for a new set of answers. Let me show you how to add it:

    1. In the Form Elements window, go to Widgets.
    2. Drag and drop the Configurable List.
    3. Click on the Wand icon next to it.

    5. You can change the label for the Add button.

    Your answer worked well the watersheds example above, as it was relatively simple, but I have another part of the survey on Payers. Similarly, I would like to ask them multiple questions about their payers, and allow them to add as many payers as they like. Only problem is the payers section is more complicated & has internal conditional logic. I may be asking for too much here, but is there a solution for this?


    Payers section (I'd like them to be able to answer all 7 questions for as many payers as they like)

    1. Payer name
    2. Payer type (drop down)
    3. If selected A above, new dropdown...
    4. If selected B above, new dropdown..
    5. If selected C above, new dropdown..
    6. Payer sector (drop down)
    7. If selected A above, new dropdown...
    8. If selected B above, new dropdown..
    9. If selected C above, new dropdown..
    10. 4th question (dropdown)
    11. 5th question (dropdown)
    12. 6th question (dropdown)
    13. 7th question (#)

    Hi Mia,

    Thanks for getting back to us. You can remove the requirement of the widget. Let me show you how:

    1. Click on the Gear icon next to the widget.
    2. In the menu on the right, scroll down and select No under Required.

    As per your question in your last message, please note that it's not possible to perform conditional logic inside the Configurable List widget. We can try to look for an alternative, can you provide more details about what you need to achieve in the payer section?

    I cloned your form and I see that you already added the fields in the Payer Section. As per your goal, it is not possible to use conditions within the Configurable List widget. So, I suggest that you stick with your current settings on your Payer Section. As for the Payer Sector field, you can use the same method as you did for the Payer Type to display the corresponding dropdown field based on the user's answer. Here's one example:
