Account is suspended due to credit card details collection

I understand my account is disabled. I need your help in fixing this

Thanks for reaching out to Jotform Support. Your account has been suspended because your Service Fee Agreement Template form is collecting credit card details using plain text fields. Here's a screenshot of the fields:

Please be advised that this is a violation of our Terms of Use. Since you have not collected any submissions yet, I have deleted the offending fields and reactivated your account.
